The Houston health department is asking area residents to offer their suggestions for making the city and Harris County communities more “livable”, according to The Houston Chronicle:
The input is needed for the “Community Transformation Initiative,” a joint Houston-Harris County project led by the Houston Department of Health and Human Services.
Area residents are asked to complete an online survey, available in English and Spanish, at http://www.gatewaytocare.org/survey by Aug. 31.
Community transformation is a nationwide movement that seeks to improve conditions where people live, work and play that will help residents make healthier choices. The initiative, which seeks to create healthier communities by making healthy living easier, is designed to address chronic diseases including heart disease, cancer, stroke and diabetes as well as reduce health inequities and control health care spending.
